Job Description
Speciality Nurse II- Diabetes & Endocrinology Clinic
Provides direct and indirect patient care in the office setting to patients, families, and caregivers within the Medical Group practice. Utilizes the nursing process consistent with acceptable standards of the profession and MVHS Medical Group. Delivers high-quality, patient-centered care reflecting initiative, flexibility, and responsibility with minimal administrative supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with outpatient clinic physicians and NP/PAs (healthcare providers) about changes in patient’s status, symptomatology, and results of diagnostic studies. Possesses the ability to respond quickly and accurately to changes in condition or response to treatment.
Core Job Responsibilities
- Supports the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need.
- Assists in specialty office procedures as directed.
- Participates in the daily huddle to identify individual patient needs for upcoming appointments.
- Acts as a clinical liaison between patient and provider for questions, concerns, and testing results.
- Communicates effectively with other members of the care team in coordinating the individualized plan of care.
- Supports PCP during in-office procedures and examinations.
- Provides outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
- Performs duties within scope of practice to the highest level of licensure.
- Provides direct support & guidance to other members of the care team as appropriate.
- Performs assessment data collection on all patients utilizing tools/forms in EMR.
- Adequately assesses and reassesses pain.
- Educates the patient and family regarding pain management.
- Provides age-appropriate care to patients in the community setting.
- Consistently documents procedures completed in the EMR, to include medications administered and follow-up assessments.
- Delivers medication utilizing the five (5) rights and observes patient for desired or untoward effects.
- Maintains current knowledge of medications and their correct administration.
- Establishes priorities of patient care based on needs and office resources.
- Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skill.
- Performs other duties as required.
Education/Experience Requirements
Required:
- Graduate of an accredited school of nursing or nursing program.
- Current New York State Registered Nursing registration.
- BLS Certification.
- Minimum 3 years of nursing experience.
- Basic computer and EMR knowledge.
- Excellent communication and interpersonal skills.
- Ability to assimilate information from many different sources.
- Must be able to communicate in English both verbally and in writing.
Preferred:
- Bachelor of Science in Nursing from an accredited nursing program or higher degree.
Licensure/Certification Requirements
Required:
- Current New York State licensure as a Registered Nurse.
