Job Description
Clinical Pharmacist
The Clinical Pharmacist will support the mission and vision of the Clinical Pharmacy team to the top of their license. This is a patient-facing role that will support the initiatives set out by the department. These initiatives align with the company’s strategic direction by focusing on improved performance on the medication related quality measures, optimizing high-cost drug utilization, and decreasing the risk of medication related problems that lead to unnecessary healthcare expenditure.

Hours & Location
- Full Time: Monday-Friday 8:30am-5:00pm
- Remote position
Essential Duties And Responsibilities
- Reviews and verifies prescriptions for patients identified as high risk to reduce any inadvertent complications due to polypharmacy; work with the case management teams, patient, and any providers to reduce medical complications or otherwise negative interactions.
- If appropriate, contacts medical providers’ offices to clarify prescriptions, dosages, refills, interactions, and allergies, and to suggest alternative medications, and answer medical provider questions.
- Support Millennium’s strategy to achieve 5 Star rating regarding quality initiatives relating to medications.
- Works as an active member of multidisciplinary team as a collaborative resource to MPG Physicians, Providers, and Care Management team.
- Adheres to government laws and regulations, corporate policies and procedures, ethics, and codes of conduct.
- Develops and maintains good relationships with medical community including physicians, nurses, and other health care providers by medical provider detailing and outreach to health groups, retirement homes, nursing homes, and other forums for enhancing growth opportunities.
- Maintains knowledge and skill in the fields of pharmacy and healthcare, reads pharmacy related journals, reads company publications and communications, completes People Plus Learning modules and continuing education credits, research new drugs, attends district meetings, etc.
- Obtains necessary certifications and completes other training required by the company.
- Promotes teamwork and cooperation among employees, including helping resolve conflict, giving feedback, recognizing progress, fostering a sense of shared success, and assists other employees as needed.
- Complete Drug Utilization Reviews for criteria-based physician orders within the organization.
- Exhibits initiative and self-direction, with excellent time management skills.
Required Knowledge And Experience
- Graduate of an ACPE accredited College of Pharmacy (Required)
- Doctor of Pharmacy or other advanced degree (Required)
- 5 years related clinical pharmacist experience or
- Completion of an ASHP accredited Pharmacy Residency or Board of Pharmacy Specialties or other related certification
Certificates, Licenses, Registrations
- Active, unrestricted Pharmacist License- Florida (Required)
- Active, unrestricted Pharmacist License – GA, NC, and TX or willing to obtain within 3 months of start date.
- Willing to obtain pharmacist license in any additional state as needed.
