Role Overview
The TOC Case Manager III role involves providing a member-centered, evidence-based model of care. Responsibilities include ensuring smooth transitions between care settings, assessing individual care goals, coordinating medical and supportive services, and connecting members with community resources to reduce illness burden and healthcare costs. Additional duties may be assigned as needed.
Required Qualifications
• Associate’s or Bachelor’s degree in Nursing and 5+ years of clinical/acute care experience.
• Current, unrestricted RN license in the state of residence.
Certifications & Experience
Certified Case Manager (CCM) is preferred or the ability and commitment to obtain within two years of hire. Experience with vulnerable and complex populations, managing computer systems, and multiple documentation platforms is highly valued.
Skills & Abilities
Strong communication skills, decision-making abilities, proficiency in Microsoft Products, and the capacity to work in fast-paced, ever-changing environments are essential. A professional demeanor, empathy, and assertiveness are required.
Other Details
This is a remote position for candidates who reside within states where Medica is registered as an employer. The salary grade ranges from $70,700 to $121,200, with a typical hiring range between $70,700 and $95,950. Additional benefits include competitive health plans, PTO, 401K contributions, and more.
