Position Overview - Case Manager
The Case Manager will work with the Caseworker Manager to identify strengths, needs, and resources in support of UCs in Post Release Services. This role includes assisting with coordinating Individual Service Plans for children to help stabilize UCs with their sponsors.
Work Location & Compensation
Location: Baltimore, Maryland
Annual Compensation: $68,640 (based on experience)

Qualifications
- BSW or equivalent in education, psychology, sociology or behavioral sciences
- Bilingual in English and Spanish or access to interpretation services
- Experience with immigrant populations, family preservation, kinship care, or child welfare preferred
- Proficient in Microsoft Office Suite and related software
- Excellent communication, organization, and ability to work independently and as a team
- Must pass pre-employment health screen and required background checks
Key Responsibilities
- Maintain a caseload of Level 1 and Level 2 UCs in the community
- Support UCs and their sponsors in identifying strengths, needs, and resources
- Coordinate Individual Service Plans and complete necessary documentation
- Implement case plans using professional judgment and teamwork
- Provide crisis intervention and client de-escalation support
- Liaise with UCs, families, sponsors, and other service providers
