Overview

Job Summary

The Case Manager will:

  • Complete intake and psychosocial assessments.

  • Maintain high professional standards and confidentiality.

Essential Functions

  • Conduct psychological, social and environmental evaluations.
  • Audit case files for accuracy and completeness.
  • Review therapist and physician evaluations.
  • Advocate for necessary client services and entitlements.

Qualifications

A Master's Degree in Social Work, Mental Health, or a related field is required. Professional licenses such as LCSW, LICSW, or related certifications are preferred. A minimum of 2-3 years experience in case management, utilization review, or discharge planning is ideal.

Physical and Additional Details

This position involves standing, walking, and lifting requirements. It is an on-site role located at 1575 Cambridge Street, with a scheduled 36-hour work week during the day shift.
