Mass General Brigham is seeking a Case Manager to join its team at Spaulding Rehabilitation Hospital Corporation. In this role, you will conduct intake interviews, assess clients' psychosocial and medical needs, and coordinate services in compliance with healthcare standards.
Job Summary
The Case Manager will:
- Complete intake and psychosocial assessments.
- Review referrals, screenings, and case files.
- Assist with discharge planning and service coordination.
- Maintain high professional standards and confidentiality.
Essential Functions
- Conduct psychological, social and environmental evaluations.
- Audit case files for accuracy and completeness.
- Review therapist and physician evaluations.
- Advocate for necessary client services and entitlements.
Qualifications
A Master's Degree in Social Work, Mental Health, or a related field is required. Professional licenses such as LCSW, LICSW, or related certifications are preferred. A minimum of 2-3 years experience in case management, utilization review, or discharge planning is ideal.
Physical and Additional Details
This position involves standing, walking, and lifting requirements. It is an on-site role located at 1575 Cambridge Street, with a scheduled 36-hour work week during the day shift.
