Overview
Mass General Brigham relies on professionals in patient care, research, teaching, and community service. As a Case Management Resource Specialist, you will assist patients with transportation, accommodations, and locating affordable support services, while working closely with unit-based case managers to coordinate discharge plans.
Job Summary
Assist patients in arranging transportation and locating affordable accommodations and support groups. Collaborate with case managers for discharge planning. Provide referrals and disseminate information on community resources.
Essential Functions
- Research community social resources
- Maintain resource files and brochures
- Collaborate with social work staff and community agencies
- Plan and implement in-service presentations
- Educate patients/families on available resources
- Assess patient and family understanding
Qualifications
Bachelor's Degree required. Hospital related experience (1-2 years preferred). Must have familiarity with medical terminology and community services, strong assessment, crisis intervention, collaboration and communication skills.
Physical Requirements
Must be able to stand, walk, and lift weights (up to 35lbs with assistive device) frequently, with occasional sitting and climbing activities.
Work Details
- Schedule: Monday - Friday, 8:30-5pm with rotating holidays and one weekend per month coverage
- Work Location: 265 Charles Street
- Shift: Day
- Employee Type: Regular; 40 hours per week

Compensation & Benefits
Hourly pay ranges from $18.22 to $26.06 with eligibility for additional comprehensive benefits, career advancement opportunities, premiums, and bonuses.
