Clinical Pharmacist - Cardiology - Maine Medical Center

Maine Medical Center is seeking a Clinical Pharmacist for their Cardiology department. This is a Per Diem position.

Job Summary

The Cardiology Clinical Pharmacists are integral members of the multidisciplinary care team, responsible for providing expertise in complex cardiovascular care. This position delivers pharmacy services for adult patients with a wide spectrum of cardiopulmonary and cardiovascular conditions, requiring strong knowledge in cardiology pharmacotherapy and contemporary, evidence-based cardiac literature.

Under the general direction of the Director of Clinical Pharmacy Services, or their designee, the Per Diem Cardiology Clinical Pharmacist practices at the top of their license across multiple cardiac care areas with flexibility to support broader operational needs as required. Team members are expected to round with medical services across several cardiology subspecialties, including the general cardiac unit, the heart failure service and unit, cardiothoracic ICU, and the cardiac ICU.

In addition to order verification and rounding responsibilities, the pharmacist manages complex high-risk cardiac pharmacotherapy plans; supports integrated drug distribution services; and contributes to academic research and teaching for students, advanced practice providers, and pharmacy and medical residents.

This position requires strong clinical judgement and effective collaboration. Pharmacists are expected to contribute meaningfully to complex care discussions, provide face-to-face patient education and remain available to offer provider and nursing support at the bedside.

Responsibilities

  • Participate in daily rounds as part of the multidisciplinary team
  • Review, verify, and optimize medication therapy for critical and non-critical cardiac patients
  • Provide consultative services that foster appropriate evidence-based medication selections
  • Collaborate with multidisciplinary care givers to monitor, assess, recommend changes to and optimize medication use
  • Provide drug information consultation to providers, nurses and other clinicians requesting clinical pharmacy services on various cardiac floors
  • Optimize heart failure medications that align with guideline directed medication therapy goals
  • Participate in ongoing quality improvement activities and quality management initiatives
  • Provide health literacy-sensitive education to patients and caregivers regarding medication use, disease-state management and prevention strategies
  • Assist in precepting students, residents and other learners
  • Develop training experiences and competencies in collaboration with other clinical staff
  • Practice in accordance with state and federal laws, rules and regulations
  • Perform other related duties as required

This position requires full understanding and active participation in fulfilling the Mission of MaineHealth Hospitals. Adherence to Code of Conduct and other policies is required.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Education: The Clinical Pharmacist role reviews medication orders for safe and effective medication use; collaborates with physicians and other health care personnel to achieve optimal patient outcomes; and formulates recommendations based on current literature and evidence-based practice.
  • License/Certifications: Current applicable state(s) license with Board of Pharmacy required.
  • Experience: Previous pharmacist experience preferred.
