Overview

MaineHealth is seeking a Case Manager - Medical Outreach to deliver case management services and help clients overcome barriers to accessing affordable health care and other essential services.

Responsibilities

Manage patient caseloads and address complex medical and social issues. Collaborate with state, federal, community, municipal, and public programs to ensure client needs are met.

Required Skills and Experience

  • Bachelor’s degree preferred in fields such as Psychology, Social Work, Nursing, or related area.
  • Licensed Social Worker preferred; valid driver’s license and reliable vehicle required.
  • Two years experience in community health with case management experience; alternatively, three years of relevant healthcare experience.
