On-site Home Visitor at Magellan Health
Magellan Health is seeking a dedicated On-site Home Visitor to provide essential support to eligible military families at Fort Carson, Colorado. This vital program focuses on supporting expectant parents and families with children from birth to 3 years of age, directly contributing to military mission readiness. By enhancing knowledge and skills for healthy relationships, we aim to provide safe, nurturing environments for children and prevent family violence.
As an On-site Home Visitor, you will offer a range of activities and services, including information and referrals, parenting programs, respite care for children, and supervised play time. Families identified as being at risk for child maltreatment and/or family violence will receive additional, intensive support from your services.
Key Responsibilities
- Provide effective, timely, and confidential services to families assessed at risk for child maltreatment and/or family violence.
- Protect the privacy and confidentiality of client information at all times.
- Identify and report suspected child abuse and neglect, as well as domestic abuse.
- Assess developmental milestones and facilitate referral procedures for special needs or developmental delays.
- Assess and strengthen adaptation to parenthood and parental capacity for problem-solving and building trusting relationships.
- Promote developmentally appropriate parenting skills, disciplinary techniques, and parent-child communication.
- Facilitate informal and formal community networks to build positive relationships and reduce social isolation.
- Develop relationships with local community and military installation officials to maximize program effectiveness.
- Utilize community-based services and networks to provide concrete support for families in crisis.
- Employ methods for screening, assessing, and addressing protective and risk factors associated with child abuse and neglect using a strengths-based, family-centered approach.
- Maintain an up-to-date tracking system for case status.
- Participate in monthly conference calls and annual training events.
Required Qualifications
- At least two (2) years of direct work experience in child abuse, domestic abuse, maternal/child health, or medical social work.
- Current and active license as an LCSW, LMFT, or RN.
- Licensed Clinical Social Workers must possess a current, valid, unrestricted clinical license and a master’s degree in Social Work.
- Registered Nurses must have a bachelor’s degree in Nursing, a current unrestricted license in a U.S. State or territory, and at least two (2) years of direct work experience in relevant fields.
- Proven home visitation experience is essential.
- Valid driver’s license, auto insurance, and ability to operate a motor vehicle.
- Working knowledge of basic computer skills, including Microsoft Word, PowerPoint, and Excel.
- Ability to operate general office equipment.
- Capability to communicate and interact effectively, in person and by telephone.
- Comfortable working under time pressure and rapidly for long periods to meet deadlines.
- Ability to travel as needed, adhering to AFSC travel policies and procedures.
- Physical ability to enter private homes and transport oneself between various locations for program activities.
