Job Summary
The Community Support Case Manager is responsible for managing and overseeing all case management activities across assigned programs. This role focuses on client-centered care and utilizes trauma-informed approaches to help individuals access housing, social services, and community resources.
Essential Duties & Responsibilities
- Program Oversight & Compliance: Review scope of work, ensure adherence, and complete program requirements.
- Client Support & Case Management: Deliver culturally responsive care including intake, assessment, care planning, and documentation.
- Administrative Support (CS II & III): Attend meetings, manage timecards, and provide back-up support.
- Quality Assurance & Networking (CS II & III): Participate in audits, outreach events, and community resource fairs.
- Additional Duties for CS III: Facilitate crisis intervention, train volunteers, lead meetings, and support team members.
Education & Experience
CS I: High School diploma with 2 years experience or bachelor’s degree with no experience. CS II: High School diploma with 4 years experience or bachelor’s degree with 2 years experience. CS III: High School diploma with 6 years experience or bachelor’s degree with 3 years experience, or a master’s degree with internship plus experience.

Work Environment & Physical Demands
The role requires moderate physical activity including standing, walking, and detailed documentation with adherence to confidentiality and HIPAA standards.
