Position Overview: Home Infusion Pharmacist
Lumicera Health Services is actively seeking a Home Infusion Pharmacist to join our dedicated team. This role is crucial for providing clinical services to patients receiving specialty infusion products and those managing bleeding disorders, particularly hemophilia. The Home Infusion Pharmacist will play a significant part in daily pharmacy operations, managing patient programs for specialty infusion medications, and maintaining vital communication with patients, caregivers, Hemophilia Treatment Centers (HTC), and physicians.
As a Home Infusion Pharmacist, you will leverage extensive clinical skills and infusion knowledge to deliver specialized infusion services to patients in their homes. Your primary goal will be to optimize patient outcomes, significantly reduce emergency department (ED) visits, and minimize hospitalizations. This position involves reviewing and dispensing specialty infusion drugs and clotting factor concentrates (CFCs), supporting initiatives based on patient and business needs as guided by the Manager, Infusion Services. You will also collaborate closely with a multidisciplinary care team, including patient care assistants, clinicians, and administrative specialists, to ensure comprehensive patient monitoring and care.
Key Responsibilities
- Review and evaluate medication orders for appropriateness, considering patient-specific factors and medical history.
- Prepare and dispense specialty infusion products and related supplies in compliance with state and federal regulations, as well as organizational policies.
- Develop comprehensive factor replacement plans and individualized perioperative plans, optimizing assay management and medication possession ratios.
- Implement therapeutic optimizations to improve treatment outcomes and manage costs by coordinating laboratory tests, ensuring complete supply consumption, reporting adverse events, and supporting inventory management.
- Develop practice guidelines, coordinating with Hemophilia Treatment Centers (HTCs) and providing staff training.
- Facilitate a seamless patient care transition by bridging communication between HTCs, the specialty pharmacy, and the patient/caregiver.
- Provide patient and caregiver consultations on the safe use of infusion therapies, particularly clotting factor concentrates (CFCs), and offer disease management education over the phone.
- Lead training and develop new processes and procedures for program implementation, basing them on evidence-based literature, data, and practice standards.
- Participate in a 24/7 on-call schedule to address urgent patient calls outside of regular business hours.
- Document and report internal and external quality issues and errors to support compliance program objectives.
- Perform other assigned duties to support pharmacy operations.
Qualifications and Requirements
- Bachelor’s degree in pharmacy or Pharm D.
- Current, unrestricted licensure as a Registered Pharmacist in your state of residence, or the ability to gain reciprocity within four months.
- 1-3 years of experience in a hemophilia treatment center (HTC) or a similar setting providing direct patient care is required.
- Demonstrated knowledge of infusion/specialty pharmacy medication order systems, infusion procedures and equipment, infusion products, and their uses and administration standards.
- Additional training in an accredited Pharmacy Residency program is preferred.
- Commitment to participating in, adhering to, and supporting compliance program objectives.
- Proven ability to consistently interact cooperatively and respectfully with colleagues.
Work Schedule
This is a part-time position, requiring 2 days/week, 9-5 pm CT. While this role is remote, Lumicera is unable to offer remote work to residents of Alaska, Connecticut, Delaware, Hawaii, Kansas, Kentucky, Maine, Massachusetts, Mississippi, Montana, Nebraska, New Hampshire, New Mexico, North Dakota, Rhode Island, South Carolina, South Dakota, Vermont, West Virginia, and Wyoming.
