Overview of the Pharmacist, Clinical Consultation Role

Due to company growth, we are seeking a dedicated Pharmacist, Clinical Consultation to join our team. This role involves conducting telephonic consultations with patients about their specialty medications and disease management. Key responsibilities include medication reconciliation, identifying drug interactions, and providing recommendations for side effect management. You will also interpret prescriptions, liaise with prescribers for clarification, and expedite order processing. The ideal candidate will meet departmental productivity goals, uphold quality standards, resolve patient issues, and adapt to changing business needs. This position requires independent work with minimal supervision, representing Lumicera with a patient-centered approach to enhance the patient experience.

Key Responsibilities

  • Provide patient education via telephonic consultations, focusing on disease state management, side effect mitigation, medication administration, storage, and therapeutic appropriateness, following a patient-centered care model.
  • Answer clinical questions from patients and caregivers.
  • Maintain detailed clinical documentation of patient consultations and interventions in the pharmacy software system, adhering to internal policies and external regulatory standards.
  • Interpret and clinically evaluate prescriptions for specialty medications.
  • Contact providers and/or patients to verify prescription details, such as drug strength, directions, and diagnosis, to expedite order processing.
  • Verify the validity of controlled substances.
  • Consult with physicians and nurses regarding pharmaceutical questions and initiate prescription transfers with other pharmacies.
  • Collaborate with physicians to convert prescriptions to generic, biosimilar, or preferred drugs when possible.
  • Verify prescription information entered into the system by order entry staff.
  • Answer clinical questions from the pharmacy team.
  • Adapt to new workflows and processes, and solve unique pharmaceutical problems.
  • Stay current with advancements in specialty pharmacy therapies, new drug approvals, and disease state treatment guidelines.
  • Provide backup support to other pharmacists and assist in training less experienced pharmacists.
  • Document and report medication errors according to company policies, and contribute to error reduction and prevention methods.
  • Adhere to all applicable federal and state laws and maintain the highest ethical standards.

Qualifications

  • Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.
  • Current, unrestricted pharmacist license in your state of residence; ongoing continuing education required to maintain active licensure.
  • A minimum of two years of experience as a practicing licensed Pharmacist is preferred.
  • Strong clinical background is essential.
  • Experience with and/or a strong understanding of Specialty Pharmacy medications is desirable.
