About The Opportunity - Field Care Manager
Libertatem Healthcare Group, part of the Sonderwell Group, is dedicated to providing high-quality, compassionate, and person-focused healthcare. We are seeking a passionate and dedicated Field Care Manager to oversee the delivery of high standards of home care services.
Your key responsibilities will include conducting client assessments and managing care plans, ensuring our services meet compliance with CIW regulations. This role involves regular and frequent travel to client's homes across our South region, including Somerset, Dorset & Hampshire, and also requires travel around the South Wales region. Some evenings and weekends may be required.
Key Responsibilities
- Develop and review personalized care plans and conduct client assessments.
- Liaise with the operational team regarding performance issues and training needs.
- Ensure services meet CIW standards, conduct audits, and maintain accurate records.
- Build strong relationships with clients and their families, addressing concerns and updating care needs.
- Collaborate effectively with healthcare professionals to ensure holistic care delivery.
Requirements
- NVQ Level 3 or 4 in Health & Social Care (or equivalent qualification).
- Proven experience in a supervisory or management role within health and social care.
- A genuine passion for delivering high-quality care, coupled with exceptional communication and organisational skills.
- Full UK driving license and access to your own vehicle for extensive travel.
Benefits
- A competitive annual salary ranging from £27,000 to £35,000, depending on skills and experience.
- This is a field-based role, with some time spent working from our Swansea office between client visits.
- Standard working hours are 9 am-5:30 pm, Monday to Friday, with flexibility required for evenings/weekends.
- Mileage paid at 45p per mile for business travel.
- 25 days of annual leave plus bank holidays.
