Role Overview: Clinical Program Coordinator RN

The purpose of this position is to provide comprehensive care coordination services to Providence Health Plans (PHP) members. These services encompass a range of critical functions to support members with acute, complex, and chronic health conditions, as well as those nearing end of life.

Key Responsibilities

  • Disease Management Programs: Educating, motivating, and empowering members to effectively manage their disease.
  • Case Management: Performing triage and referral, transition of care planning, end-of-life care planning, and managing other acute and catastrophic cases for members and their families with complex health care needs.
  • Care Management Services: Providing nurse education, general care coordination, and assistance with daily functional needs.
  • Benefit Navigation: Assisting with the management of member health plan benefits and identifying alternative services when benefits are exhausted.

This position operates within the health plan framework, focusing on managing medical expenses while simultaneously improving access to and quality of care for our members.

Work Arrangement

Providence Health Plan welcomes 100% remote work for applicants who reside in the following states:

  • Washington
  • Oregon

Required Qualifications

  • 5 years Clinical nursing experience.
  • Upon hire: Current unencumbered Registered Nurse License in state of residency.
  • Experience working with physicians in the collaboration and management of patient care.

Preferred Qualifications

  • Bachelor's Degree in Nursing or health education.
  • Current nursing experience in the following areas: cardiology, endocrinology, pediatrics, obstetrics, oncology, respiratory, health education.
