About the Role: Pharmacist
As a Pharmacist at Legacy Health, you are the trusted expert patients rely on to understand their medications and use them safely and effectively. You play a vital role in patient outcomes and overall well-being by ensuring they are informed and confident in their drug therapy.
Responsibilities
CLINICAL DUTIES
Optimal patient care outcomes from drug therapy are ensured through comprehensive clinical pharmacy activities. These activities are age-appropriate for neonates, pediatric patients, adolescents, adults, and geriatric patients, in direct association with or in support of health care providers. The Pharmacist will:
- Demonstrate clinical competence by completing assigned clinical competency modules in the electronic learning system.
- Review and evaluate individual patient medication profiles and clinical status for therapy appropriateness.
- Ensure medication therapy aligns with Legacy or health insurance formulary and medication use guidelines.
- Assess patients comprehensively using standardized electronic health record (EHR) tools.
- Perform regular re-evaluation of medication therapy to achieve therapeutic goals and proactively intervene on medication-related issues.
- Perform medication therapy management and monitoring based on Legacy-specific Collaborative Practice Protocols (CPP).
- Document interventions, recommendations, and protocol-driven therapy changes in the EHR.
- Monitor, document, report, and manage adverse drug events.
- Provide accurate and timely medication information to health care staff and patients.
- Serve as a trainer for new pharmacy staff.
- Serve as a preceptor for pharmacy students and residents.
- Perform other duties as assigned.
DISTRIBUTIVE DUTIES
Safe and accurate distributive services are ensured through standardized, efficient medication use processes. The Pharmacist will:
- Ensure all medication orders are reviewed for appropriate dispensation during the assigned shift.
- Ensure appropriate use of applicable pharmacy technical systems, equipment (including environmental monitoring), and tools.
- Oversee preparation of medication doses and compounding of sterile and non-sterile products.
- Perform all assigned work procedures without compromising quality.
- Provide supervision to technicians, students, and interns as guided by state law.
- Serve as a trainer for new pharmacy staff.
- Serve as a preceptor for pharmacy students and residents.
- Use time productively to assist team members as workload allows.
- Fill outpatient prescriptions as required.
- Work with insurance companies/third-party payers to resolve reimbursement and eligibility issues for patients where applicable.
- Perform other duties as assigned.
COMMUNICATION
The Pharmacist will:
- Effectively communicate information to patients and health care providers in oral, written, and electronic formats.
- Foster teamwork through timely, open, and respectful communication.
- Demonstrate good listening skills.
- Provide education to patients.
- Provide excellent customer service.
PROFESSIONAL DEVELOPMENT
Pharmacy knowledge and skills are updated, shared, and applied continuously to improve the quality of pharmaceutical care. The Pharmacist will:
- Maintain clinical and distributive knowledge and skills appropriate to the patient populations and areas served.
- Maintain knowledge of internal practice changes (e.g., drug shortages, new protocols) that may impact clinical and/or distributive services.
- Participate in preceptor education as applicable to the precepting role.
- Maintain licensure in good standing with the state licensing body.
SAFETY AND SECURITY
Patient, personnel, department, and facility safety and security are ensured through knowledge and application of established policies and procedures, and safety, security, and emergency plans. The Pharmacist will:
- Follow all controlled substances policies and procedures.
- Complete annual mandatory self-learning modules that include safety, security, and emergency programs required by the department or health system.
- Ensure a clean, safe, and orderly environment during the work shift.
- Comply with all laws, regulations, scope of practice guidelines, and Legacy standards in providing patient care.
Qualifications
Education:
- Graduate of an accredited School of Pharmacy.
Licensure:
- Current Registered Pharmacist License (RPh) for the applicable state required.
- BLS for Provider certification from the American Heart Association required within 3 months.
