Specialty Pharmacy Pharmacist, Comprehensive Medication Management at Keck Medicine of USC

The Specialty Pharmacy Pharmacist, Comprehensive Medication Management provides comprehensive pharmaceutical care through integrated drug distribution, clinical services, and education. This role also delivers comprehensive medication management services, including telephonic and video-telehealth services, to various populations served through Keck Medicine of USC Specialty Pharmacy partnerships.

Essential Duties

  • Responsible and accountable for providing pharmaceutical services in accordance with regulatory requirements (Joint Commission, Title 22, OBRA), professional standards (ASHP guidelines), and state and federal laws.
  • Provides Comprehensive Medication Management (CMM) for Specialty pharmacy disease states and ambulatory conditions.
  • Conducts CMM consultations in person or via telemedicine for Specialty Patients.
  • Manages the requisitioning of pharmaceuticals and biologics.
  • Performs clinical verification and dispensing of Specialty drug products.
  • Delivers other clinical pharmacy services aligned with specialty pharmacy priorities, such as medication adherence, prescription clinical review, order verification, and clinical documentation.
  • Offers consultative services to patients, providers, and external facilities.
  • Coordinates with providers, patients, and the Specialty Pharmacy to ensure referrals are created per pharmacy protocol.
  • Assists in planning, coordination, and execution of pharmacy operations and programs to enforce state and federal laws and regulations pertaining to drugs.
  • Works under minimal supervision, with extensive latitude for initiative and independent judgment.
  • Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes, including advising physicians on drug therapy, toxicity, side effects, and appropriate dosing.
  • Coordinates activities of assigned pharmacy technicians involved in medication preparation, labeling, and delivery.
  • Ensures compliance with federal, state, and local laws/regulations for drug dispensing, controlled drugs, and record maintenance.
  • Mentors pharmacists in training, residents, and interns through practical and didactic experiences.
  • Maintains liaison relationships with medical and nursing staff, providing timely information on pharmaceutical supplies, drug usage/compatibility, regulations, and Joint Commission Standards.
  • Individualizes medication regimens using sound principles of pharmacodynamic and pharmacokinetic variations.
  • Responsible for establishing and continually improving Pharmaceutical Care delivery to patients within direct responsibility and assisting others.
  • Performs other related duties incidental to the work described herein.

Required Qualifications

  • Doctorate Pharmacy (Pharm.D) degree from an accredited School of Pharmacy.
  • 2 years of experience in Ambulatory Care, Specialty Pharmacy, Comprehensive Medication Management (CMM) or a similar setting.
  • Strong organization and time management skills.
  • Excellent analytical, problem-solving, planning, and evaluation skills.
  • Demonstrated ability to effectively work with physicians, staff, and patients.
  • Ability to function independently and as a member of a team.
  • Committed to excellence in patient care and customer service.

Required Licenses/Certifications

  • Registered Pharmacist (CA DCA) Current pharmacy licensure with California State Board of Pharmacy.
  • Basic Life Support (BLS) from American Heart Association.
  • Advanced Cardiovascular Life Support (ACLS) from American Heart Association.
  • Advanced Practice Pharmacist (APP) via the CA State Board of Pharmacy required or obtained within 1 year of hire.
  • Fire Life Safety Training (LA City): If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before the expiration date (Required within LA City only).
