Home Health Nurse - Kaleida Health
Kaleida Health is seeking a dedicated and compassionate Home Health Nurse to join our VNA Team 2 in Erie County. This is a full-time, onsite position focused on providing comprehensive patient care in the home setting.
Key Responsibilities
- Coordinate the total Plan of Care for patients in the Home Care setting.
- Assess, plan, implement, and evaluate patient care.
- Provide leadership for the Health Care team by collaborating with other professionals.
- Maintain standards of professional nursing practice in the Home Care setting.
Qualifications
- Associates Degree required.
- Current RN license required.
- 1-5 years of relevant experience required.
Work Environment and Schedule
- Location: Home Care-VNA Erie County, Cheektowaga, NY.
- Work Type: Full-Time.
- Shift: 8a-4:30p.
- Standard Hours Bi-Weekly: 80.25.
- Requires working some weekends and holidays.
- On-Call is required with rotation.
- Work Arrangement: Onsite.
Compensation and Benefits
- Salary Range: $42.23 - $52.68 per hour (Bi-Weekly Pay Frequency).
- Wage determined by experience, qualifications, internal equity, and collective bargaining agreement.
