Job Summary
Under indirect or direct supervision, the Inpatient Pharmacy Specialist manages and monitors medication therapy for hospitalized or ambulatory patients. The specialist confers with and assists medical personnel, provides medication information and pharmacokinetic dosing advice, adjusts doses, orders laboratory tests, administers medications, monitors medication distribution, and directs ancillary pharmacy staff.
Essential Responsibilities
- Uphold Kaiser Permanente policies and procedures, state, federal, and local laws.
- Perform sterile and non-sterile compounding.
- Develop and implement patient-specific plans to optimize medication therapy.
- Monitor medication therapy for optimal therapeutic outcomes.
- Follow hospital approved collaborative pharmacokinetic drug therapy management protocols.
- Interview patients/families to obtain medication use history.
- Document patient information in medical records or computer systems.
- Advise patients, families, and medical personnel on proper medication use, storage, and safety.
- Review medication orders for adherence to drug formulary and intervene when necessary.
- Ensure proper medication storage, security, and accuracy in preparation and dispensing.
- Participate in investigational drug studies, quality management programs, and medical emergencies.
- Develop and deliver education and training programs for pharmacy and medical personnel.
Experience & Education
Minimum of 1200 hours of inpatient pharmacy experience within the last 5 years or completion of an ASHP accredited clinical pharmacy practice residency. A degree in Pharmacy meeting minimum education requirements for licensure is required.
License, Certification, Registration
- Controlled Substance Utilization Review and Evaluation System Registration required at hire
- Pharmacist License (California) required at hire
- National Provider Identifier required at hire
- Advanced Cardiac Life Support and Basic Life Support within 3 months of hire
Additional & Preferred Qualifications
Candidate must demonstrate comprehensive knowledge of acute care medications, pharmacological principles, and aseptic technique for sterile product preparation. Excellent communication skills and proficiency with pharmacy computer systems are essential. A Doctorate of Pharmacy (Pharm.D.) is preferred.
