Jobgether is seeking a dedicated Remote Nurse Case Manager on behalf of a partner company. In this crucial role, you will empower patients throughout their medical journeys, ensuring they access essential care and resources. You will manage diverse patient caseloads, collaborate effectively with healthcare providers, and lead vital training for team members. Key Accountabilities
- Manage an assigned patient caseload, consistently meeting inbound and outbound call targets.
- Coordinate patient reimbursements and appointment scheduling efficiently.
- Provide expert clinical support and guidance through the patient enrollment process.
- Assess the feasibility of requests for medication administration, ensuring safety and compliance.
- Maintain accurate and current documentation for all patient appointments.
- Lead disease-state training initiatives for both new and existing team members.
- Offer valuable feedback on established protocols and inter-departmental relationships.
- Report all Adverse Events promptly to the appropriate units or departments.
Requirements
- Hold a Registered Nurse (RN) or Registered Practical Nurse (RPN) diploma.
- Possess a minimum of five (5) years of progressive Nursing experience.
- Previous healthcare and case management experience is strongly preferred.
- Bilingualism (French and English) is a significant asset.
- Experience in oncology is considered an advantage.
- Demonstrate a strong patient-focused approach and excellent critical thinking abilities.
- Exhibit effective communication skills, both verbal and written.
- Proven ability to multi-task effectively and perform well under pressure.
- Proficiency in Microsoft Office suite (Word, Excel, Outlook).
