Bilingual Care Management Support

This fully remote role supports members in taking meaningful steps toward better health and well-being. You will connect with members by phone, complete non-clinical assessments, identify barriers to care, and help individuals access appropriate resources and benefits. Working in a fast-paced call center environment, you will manage both inbound and outbound interactions while maintaining accurate documentation across multiple systems. The position combines compassionate member service with structured processes, productivity standards, and close collaboration with care teams. Success requires strong communication, sound judgment, attention to detail, and the ability to work independently.
