Specialty Pharmacist I at Jefferson Health
As a Specialty Pharmacist I at Jefferson Health, you will be responsible for interpreting physicians' prescriptions and dispensing high-dollar specialty and other medications, ensuring strict adherence to regulatory, accreditation, and policy guidelines. A core component of this role involves providing comprehensive medication therapy management, actively managing drug therapy to identify, prevent, and resolve medication-related problems. This includes counseling patients and medical staff, and proficiently handling problem resolution.
You will meticulously complete patient records, initial assessments, medication profiles, initial fill notes, and ongoing progress notes. Participation in coordinating patient care is essential to meet desired outcomes and goals, covering aspects such as initial referrals, medication review and selection, assessment of medication delivery systems, coordinated care conferences, patient care plans, compliance programs, and patient discharge processes.
The role also requires interaction with internal and external clinical and non-clinical personnel to support benefits investigation, financial assistance, and prior authorization needs. You will be instrumental in activities aimed at mitigating patient side effects and promoting therapy adherence. Furthermore, you will engage in practice pull-through for specialty and other orders and contribute to the development and maintenance of the patient management program. This position offers the opportunity to participate in and lead initiatives and projects that significantly impact critical clinical, operational, and financial areas.
Essential Functions
- Applies practical knowledge of the job area, typically obtained through advanced education, specialty pharmacy certification, or relevant work experience.
- Works independently with general supervision.
- Addresses difficult but typically not complex problems.
- May influence others within the job area by explaining facts, policies, and practices.
- Coaches and reviews the work of technical staff.
- Interacts with co-workers, visitors, and other staff consistent with Jefferson's iSCORE values.
Other Functions and Competencies
- Documents all interactions with patients and prescribers in appropriate systems, ensuring compliance with protocol.
- Performs On-Call activities as required.
- Adheres to all pharmacy, state, and federal laws and regulations.
- Performs other duties as assigned.

- Specialty Pharmacy Representatives I and II
- Rotation IPPE and APPE students
Educational/Training Requirements
- PharmD degree with at least 3 years of Pharmacy experience or 1-2 years of Specialty Pharmacy experience is preferred.
Certificates, Licenses, And Registration
- Valid Pharmacist licensure in the State of Pennsylvania.
Experience Requirements
- Prior specialty pharmacy experience or residency preferred.
