Per Diem Pharmacist

Job Details

The Per Diem Pharmacist provides essential patient care activities including conducting patient rounds with physicians, comprehensive patient monitoring, basic pharmacokinetics, and drug utilization evaluations. This role actively participates in the department's performance improvement program. Additionally, the pharmacist assists in delivering drug information services within the department and contributes to the education of pharmacy staff, other healthcare professionals, and the community on drug-related topics. All duties are performed in support of the Jefferson Health mission, ensuring the highest quality of patient care in an economically sound and efficient manner.

Qualifications

The successful incumbent must demonstrate the potential ability to perform the essential functions of the job. Authorization or qualification for authorization as a preceptor of student interns in Pharmacy, as described by the State Board of Pharmacy, is required. Membership in both state and national hospital pharmacy organizations is preferred, along with demonstrated human relations and effective communication skills.

Education

• Doctor of Pharmacy Degree (PharmD) is required.

Experience

• Clinical experience as a pharmacist in an acute care setting is required.
• Completion of a general or specialized residency is preferred.

Licensure/Certification/Registration Requirements

• Current licensure as a Pharmacist in the state of Pennsylvania is mandatory.
