Job Description
The Registered Nurse Long Term Care position will support the US Department of Veterans Affairs Critical Staffing Program. The individual is expected to provide ongoing, holistic care in long-term care facilities by developing and implementing individualized care plans, administering medications, and collaborating with an interdisciplinary team. Responsibilities include comprehensive resident assessments, medication administration, monitoring health status, and assisting with daily living activities to promote independence and dignity.
- Conduct assessments to identify residents’ physical, emotional, and social needs.
- Develop, implement, and evaluate individualized care plans.
- Administer medications and treatments in compliance with standards.
- Monitor health status and report changes to the medical team.
- Support and educate residents and families on health management.
- Facilitate communication among team members for continuity of care.
- Participate in quality improvement initiatives and comply with policies.
- Complete required organizational compliance education.
