Position Overview: Care Team Assistant
This remote position (CST required) operates on a 3x12s schedule (7:00am - 7:00pm CST), either Monday-Wednesday or Wednesday-Friday. The Care Team Assistant offers crucial clinical and clerical support, ensuring the delivery of high-quality, compassionate, evidence-based care within a virtual Value-Based Care framework.
What You'll Do
- Accept and return patient phone calls regarding clinical inquiries for providers, performing protocol-based triage as appropriate. Route these calls as phone notes in the EHRS for provider review and instructions.
- Place calls and generate template-based letters, as directed by providers, to deliver all lab results to patients promptly.
- Daily review schedules to confirm all pre-visit preparations are complete. This includes ensuring lab and imaging results, consultation reports, care transition details, or significant health status changes are present in the patient's medical record. If documents are missing, proactively contact patients, consultants, imaging centers, and hospitals to arrange timely delivery.
- Call patients at risk for "no-show" appointments, as well as new patients establishing care for onboarding visits, reminding them of responsibilities. Contact "no-show" patients to assess their status, reschedule needs, identify care barriers, and initiate appropriate social service referrals.
- Initiate the completion of forms (e.g., disability forms, home health orders, DME requests, disease status letters) for provider review and final signature.
- Assist the care team in regularly completing reports to manage and assess the patient population's health needs. Help identify "vulnerable population" members and provide suitable social service or community-based referrals.

- High-school level education or equivalency.
- Medical Assistant Certification or equivalent, OR at least 2 years of clinical experience in an outpatient setting.
- Spanish fluency preferred.
