About the Role
Ignite Medical Resort Round Rock is seeking a full time Director of Care Transitions / Social Services Director. This leader will play a vital role in enhancing the resident and guest experience by ensuring smooth care transitions and high quality social services.

Essential Duties & Qualifications
This role requires a minimum of one year working with the elderly in an acute, long-term care, or community-based setting. The ideal candidate is compassionate, able to build rapport with residents and families, and capable of making independent decisions.
- One year experience with elderly populations
- Strong communication skills in English
- Independent decision-making ability
- Demonstrates progressive leadership and initiative
Physical Demands
The role requires regular sitting, standing, bending, walking, and the ability to lift up to 25 pounds. Adequate visual and auditory abilities are necessary for effective communication.
