About the Role

Join Humana as a Case Manager and make a real difference within the Inclusa/Humana team, serving members in the Wisconsin Family Care (FC) program. In this dynamic role, you will deliver exceptional care and empower members to thrive through comprehensive social service care management for frail elders and adults with intellectual, developmental, or physical disabilities.

Key Responsibilities

  • Assess members to identify strengths and health/safety needs.
  • Develop and update comprehensive Member Care Plans.
  • Coordinate services and collaborate with Field Care Nurses.
  • Conduct in-person visits and maintain monthly phone contact.
  • Arrange and monitor community support services.
  • Document activities, case notes, and service updates accurately.
  • Travel to member residences and service locations.

Required Qualifications

A four-year bachelor’s degree in human services or a related field with relevant experience, or an alternative degree with additional experience. Intermediate computer skills including Microsoft Office are required.

Preferred Qualifications

  • Previous experience in case management.
  • Experience with electronic case note documentation.
  • Knowledge of community health and social service resources.

Additional Information

This field position requires travel (up to 40%) within the Chippewa/Eau Claire area in WI. The role supports a remote work arrangement with occasional travel for training or meetings. It includes adherence to safety standards, TB screening, and participation in the HireVue interview process.

Work Hours and Compensation

Scheduled weekly hours are 40 with a typical workday from Monday – Friday, 8:00 am to 4:30 pm CST. The pay range is between $53,700 and $72,600 per year.
