What To Expect (Job Responsibilities)
- Monitor RPM vitals daily and initiate patient outreach for abnormal readings.
- Log asynchronous CCM time in eClinicalWorks for care coordination activities.
- Moderate the weekly virtual cardiac exercise class, ensuring patient safety and documentation.
- Oversee AI-assisted onboarding workflows and verify the accuracy of patient charts.
- Coordinate RPM device inventory and ensure proper linkage for new patients.
What Is Required (Qualifications)
- Medical Assistant certification (CMA, RMA, or equivalent).
- Minimum of 2 years of experience as a Medical Assistant in a clinical, telehealth, or care management setting.
- Proficiency in eClinicalWorks (eCW) for documentation and care plan management.
- Working knowledge of chronic disease management programs, specifically CCM and RPM.
- Experience in cardiology or cardiac-focused telehealth settings is a strong plus.
How To Stand Out (Preferred Qualifications)
