Position Summary
The Pharmacist, PR will be instrumental in developing, implementing, and managing programs designed to ensure that medications for kidney disease and related comorbidities are both evidence-based and cost-effective. This role requires close collaboration with physicians, advanced care practitioners, nurses, and other clinicians to provide expert guidance on medication stewardship, with a specific emphasis on kidney disease and prevalent comorbidities. The clinical pharmacist will lead Healthmap’s comprehensive pharmacy strategy and work cross-functionally to add value to all pharmacy-related initiatives and clinical programs.
Responsibilities
- Review medication regimens and propose adjustments based on protocols and current guidelines.
- Create educational materials for providers, members, and staff on new drugs, indications, guideline updates, and industry counter-detailing.
- Participate in the development of treatment algorithms, guidelines, policies, and procedures for kidney disease management.
- Evaluate high-cost, high-risk, or high-volume medications using a population health framework.
- Optimize disease management for members.
- Enhance processes for medication reconciliation, adherence, and the reduction of avoidable polypharmacy.
- Conduct drug use evaluations and disease-based audits against predetermined criteria.
- Collaborate with other care providers to effectively share practice expertise.
- Communicate clinical information accurately and comprehensively to other patient care providers.
- Adhere to policies, procedures, and state/federal regulations to ensure compliance and confidentiality of Protected Health Information (PHI).
- Participate in mandatory Compliance, HIPPA, and other important trainings aligned with Healthmap’s HR Security Policies.
- Perform other duties as assigned.
Requirements
- PharmD degree from an accredited School of Pharmacy with an ASHP Accredited Residency.
- 3 years of clinical pharmacy experience, including direct interaction with physicians and healthcare providers in hospital or ambulatory care settings.
- Active Pharmacy License in Puerto Rico.
- Experience in renal patient pharmacy management and value-based care is preferred, as is Health Plan and/or Vendor experience.
- Understanding of Centers for Medicare and Medicaid Services (CMS) Star measures, HEDIS measures, MIPS, and National Quality Forum measures.
- Bilingual fluency in English and Spanish.
- Must reside in Puerto Rico.
