Overview

About the Role

The Clinical Oncology Pharmacist at Fred Hutch is integral to patient care, delivering pharmacotherapeutic recommendations to ensure safe and effective drug therapies. The role involves direct patient care, drug dispensing, research & education, and practice management.

Responsibilities

  • Collaborate with providers to individualize drug therapies.
  • Perform comprehensive medication therapy reviews and ensure safe transitions of care.
  • Monitor and modify drug regimens with laboratory testing and assessments.
  • Provide refill authorizations and act as a drug information resource.
  • Assist specialty pharmacy team for patient access to medications.
  • Educate patients on self-management and adherence to regimens.
  • Manage therapies such as anticoagulation, infectious disease and palliative care under collaborative agreements.

Education & Research

  • Precept pharmacy residents and student pharmacists.
  • Deliver education to medical staff and students.
  • Participate in collaborative research projects and publications.
  • Assist in drug study protocols and investigational drug studies.

Practice Management & Dispensing

  • Implement departmental policies for medication safety.
  • Develop drug therapy monitoring systems and quality assurance programs.
  • Participate in pharmacy committees, drug evaluations, and cost analyses.
  • Perform dispensing duties as needed in oral chemo, infusion or retail pharmacy.

Qualifications

  • Graduation from an accredited School of Pharmacy.
  • State of Washington Pharmacist License.
  • Minimum two years of pharmacy practice experience or equivalent residency experience.
  • Doctor of Pharmacy (PharmD) preferred.

Additional Information

This is a Per-Diem role supporting the Northwest, Overlake, and Evergreen locations. Fred Hutch values collaboration, compassion, determination, excellence, innovation, integrity, and respect. Pre-employment drug screening is required. The hourly pay range is $80.96 to $134.93 based on experience and qualifications. No immigration sponsorship is provided.
