Your Role: An Essential Link Between Hospital and Home

As a Hospital Liaison Nurse, you become a key player in the link between conventional hospital care and home hospitalization for the patient. Every day, you help make a smooth, secure, and personalized return home possible.

Your Key Responsibilities

• Be the privileged point of contact for hospital departments.
• Propose innovative discharge solutions, with the coordinating physician and social worker.
• Develop HAD activity in your territory, participating in its outreach and partnerships.
• Assess clinical situations and care needs closest to the patient.
• Ensure optimal coordination of means, care, and resources.
• Fully participate in the clinical analysis of complex situations.

The Ideal Profile

Your Profile?

You hold a State Nursing Diploma and ideally have a DU in Healthcare Pathway Coordination, with experience in home care and knowledge of palliative care.

You demonstrate strong interpersonal skills, a sense of partnership, and enjoy challenges.
