Your Role: An Essential Link Between Hospital and Home

As a Hospital Liaison Nurse, you will be a key player in the connection between conventional hospital care and the patient's home hospitalization. Every day, you will help make a smooth, safe, and personalized return home possible.

Your Key Responsibilities

• Be the privileged contact for hospital departments.
• Propose innovative discharge solutions with the coordinating physician and social worker.
• Develop the HAD activity in your territory by participating in its outreach and partnerships.
• Assess clinical situations and care needs as close as possible to the patient.
• Ensure optimal coordination of resources, care, and means.
• Fully participate in the clinical analysis of complex situations.

The Ideal Candidate

You hold a State Diploma of Nursing and ideally have a DU in Care Pathway Coordination, with experience in home care and knowledge of palliative care. You demonstrate strong interpersonal skills, a sense of partnership, and enjoy challenges. We will support you upon your arrival with an institutional integration program where you will meet HR, support departments, and the teams you will collaborate with.
