Job Description
Elwyn is seeking a Case Manager responsible for the daily care of program clients. You will provide counseling and advocacy in a therapeutic setting to assist clients ranging from young adults to geriatric individuals with chronic mental illness and co-occurring disorders. The goal is to help clients achieve social, economic, emotional, and personal independence to live in the community.
Duties and Responsibilities
- Identify signs and symptoms of mental illnesses and intervene therapeutically.
- Monitor and manage prescribed medications per state-specific guidelines.
- Maintain client records according to agency policy.
- Develop, implement, and modify treatment plans based on assessments.
- Ensure client environments are safe and clean.
- Provide psychoeducational services to clients and families.
- Meet productivity standards in fee-for-service programs.
- Integrate the Prism Model for effective treatment practices.
- Perform CPR, crisis intervention, and medication administration following protocols.
- Maintain current state-mandated training and certifications.
- Provide on-call coverage and respond promptly to staffing needs.
Qualifications, Education, and Experience
- Associate or bachelor’s degree required; degree in Social Work, Psychology, or related field preferred.
- Experience with unionized workforce and nonprofit human services is a plus.
- Strong teamwork, customer interaction, and communication skills required.
- Ability to prioritize in a fast-paced environment with strong attention to detail.
- Proficiency in Microsoft Office, familiarity with EHR systems preferred.
- Valid driver's license with acceptable driving record and minimum two years driving experience required.
