Job Description

Elwyn is seeking a Case Manager responsible for the daily care of program clients. You will provide counseling and advocacy in a therapeutic setting to assist clients ranging from young adults to geriatric individuals with chronic mental illness and co-occurring disorders. The goal is to help clients achieve social, economic, emotional, and personal independence to live in the community.

Duties and Responsibilities

  • Identify signs and symptoms of mental illnesses and intervene therapeutically.
  • Monitor and manage prescribed medications per state-specific guidelines.
  • Maintain client records according to agency policy.
  • Develop, implement, and modify treatment plans based on assessments.
  • Ensure client environments are safe and clean.
  • Provide psychoeducational services to clients and families.
  • Meet productivity standards in fee-for-service programs.
  • Integrate the Prism Model for effective treatment practices.
  • Perform CPR, crisis intervention, and medication administration following protocols.
  • Maintain current state-mandated training and certifications.
  • Provide on-call coverage and respond promptly to staffing needs.

Qualifications, Education, and Experience

  • Associate or bachelor’s degree required; degree in Social Work, Psychology, or related field preferred.
  • Experience with unionized workforce and nonprofit human services is a plus.
  • Strong teamwork, customer interaction, and communication skills required.
  • Ability to prioritize in a fast-paced environment with strong attention to detail.
  • Proficiency in Microsoft Office, familiarity with EHR systems preferred.
  • Valid driver's license with acceptable driving record and minimum two years driving experience required.
