Job Purpose
The Program Coordinator – Primary Care MAT (RN) will work as part of El Rio’s integrated behavioral health-primary care MAT program team to support all facets of care for MAT patients. The Nurse is an initial point of contact for new patients and supports continued care coordination with primary care and behavioral health services.
Essential Job Functions
- Contact patients referred to the MAT program to assess needs.
- Serve as the primary ongoing contact for MAT primary care patients.
- Ensure non-judgmental care using motivational interviewing and assessments.
- Maintain patient records, registries, and follow-up documentation.
- Collaborate with MAT providers and interdisciplinary team members for care logistics.
- Coordinate patient transitions between inpatient detox, crisis services, and MAT services.
Minimum Education and Experience
A degree or diploma as a Registered Nurse from an accredited school with a minimum of three years of hands-on patient care and experience working with patients facing substance use disorders. One year of care for patients seeking addiction treatment is required.
Licenses, Certifications, and Requirements
- Current RN license in good standing from the Arizona Board of Nursing
- Basic Life Support (BLS) certification
- Valid Level I fingerprint clearance card (or application within 7 days of hire)
- Reliable transportation and valid driver’s license complying with Arizona requirements
Preferred Qualifications
Five years of postgraduate experience with addiction service delivery, one year in care coordination, experience in a primary care team, crisis intervention training, and bilingual capability (English/Spanish).
