About the Role
The Clinical Associate is an integral part of our interdisciplinary virtual and home-based medical practice called Devoted Medical (DM). Devoted Medical delivers comprehensive wellness visits, transitions of care visits, monthly longitudinal visits for complex members, and on-demand urgent care visits. This role may involve occasional weekend hours through optional overtime.
As a Clinical Associate, you will be a liaison between our healthcare services and Devoted members. You will work closely with our clinicians to provide high-touch telehealth visits and support our clinical team by identifying care gaps and coordinating care across a fragmented healthcare system. This includes communicating with PCP offices, coordinating pharmacy services, and managing durable medical equipment (DME), home health care, imaging, and laboratory testing needs. Your goal is to ensure every member receives the care you would want for a loved one, leading to better health outcomes and a superior care experience.
Responsibilities and Impact
Responsibilities will vary based on the clinical program (Devoted Medical, Devoted2Me, Specialty Care - Hypertension, Responsive Care).
Pre-Visit Prep
- Preparing members for telehealth visits, ensuring they are comfortable with video onboarding.
Post-Visit Prep and Order Management/Care Coordination
- Coordinating with home health partners for DME and skilled nursing services.
- Arranging lab testing and imaging, diligently tracking results for clinician review.
- Notifying members of lab results and coordinating follow-up care as directed by the clinician.
- Collaborating with DM Providers on medical record collection, information gathering, appointment scheduling, and outbound communications. (Note: This role does not involve clinical decision-making or activities requiring licensure.)
Patient Education and Support
- Serving as an advocate for members by answering and returning calls for care coordination issues.
- Reviewing internal program referrals and sending welcome kits and educational materials.
Work Schedule
Available Schedules: Monday - Friday 8:30 AM–5:00 PM ET, 9:00 AM-5:30 PM ET, 10:00 AM-6:30 PM ET, 11:30 AM-8:00 PM ET. Schedules will be discussed during the interview process.
Required Skills and Experience
- MA Certification (required, expired OK). If no certification, significant MA experience may be considered with diploma and transcripts.
- Fully bilingual (English/Spanish).
- Two (2) years of patient-facing experience, telehealth experience is a plus.
- Prior primary care experience is a plus.

- Experience in care coordination, especially for Medicare patients to close Stars care gaps and complete preventative screenings.

Salary Range
$23.00 - $27.00 / hour. The actual base salary will depend on qualifications, experience, skills, education, certifications, and location.
