About the Role
As a Registered Nurse (RN) Case Manager at DaVita Kidney Care, you will play a crucial role in coordinating and facilitating quality, cost-effective care for End-Stage Renal Disease (ESRD) and Chronic Kidney Disease (CKD) patients. You will work autonomously and collaboratively with the healthcare team, patients, families, physicians, and community resources to minimize fragmentation in the healthcare delivery system.

What You'll Do
- Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients.
- Collaborate with patients, families, physicians, providers, and care teams.
- Act as a liaison between patients, families, and various healthcare services.
- Manage care to achieve clinical, financial, and patient satisfaction outcomes.
- Function as a change agent to improve healthcare delivery processes.
Qualifications
- ADN degree from an accredited school of nursing required; BSN preferred.
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of five (5) years of experience in clinical nursing strongly preferred.
- Minimum of three (3) years of experience in renal nursing preferred.
- Case management experience required.
- Demonstrated knowledge of clinical and financial outcomes.
- Previous experience in healthcare performance coaching required.
- Current CPR certification required.
- Valid driver's license with a positive driving record.
- Proficiency in basic computer skills, MS Word, and Outlook.
- Functional proficiency with DaVita specific clinical software programs (e.g., Capella) within 90 days.
- Dedicated home office with reliable internet connectivity (minimum 1MB upload/download).
