Forms Specialist Role Overview
This is a Remote role Full-Time: Monday - Friday, 8 am - 430 pm EST. You will be comfortable working in a high-volume production environment, processing medical record requests. This involves taking calls from patients, insurance companies and attorneys to provide medical record status, and accurately documenting information in multiple platforms using two computer monitors. Proficiency in Microsoft Office (including Word and Excel) is essential.

Compensation Details
Pay ranges for this job title may differ based on location, responsibilities, skills, experience, and other requirements of the role. The estimated base pay range per hour for this role is: $16—$20.50 USD.
Health and Safety Requirements
To ensure the safety of patients and staff, many of our clients require post-offer health screenings and proof and/or completion of various vaccinations such as the flu shot, Tdap, COVID-19, etc. Any requests to be exempted from these requirements will be reviewed by Datavant Human Resources and determined on a case-by-case basis.
