Overview of the Nurse Practitioner Role
ComplexCare Solutions (CCS) is seeking a dedicated Nurse Practitioner to provide high-value in-home and virtual assessments for health plans. This per diem position covers all zip codes in Kings County, NY. We focus on comprehensive member engagement, evaluating health status, addressing care gaps, identifying health risks, and managing care to improve clinical outcomes and keep members healthy at home. Our model allows Providers a full hour with each member to understand their unique needs.

Compensation & Benefits
For each completed in-home assessment, compensation is $120. (Telehealth assessments, if available, are compensated at $85 per completed assessment).
This position offers eligibility to participate in a company 401K plan, with automatic enrollment at a 3% contribution rate, providing a valuable opportunity for retirement savings.
Key Responsibilities
- Demonstrate knowledge of CMS Regulations, NCQA HEDIS Guidelines, and Evidence-based Clinical Practice Standards (ADA, ACC, AHA).
- Exhibit familiarity with ICD-10 and CPT-4 coding practices.
- Complete comprehensive, accurate, and timely in-home and virtual assessments for the assigned member population, ensuring prompt submission of all required documentation.
- Document all pertinent and active medical conditions in compliance with CMS/DHHS, company policy, and client requirements.
- Support the physician/patient relationship by ensuring timely communication of assessment findings, recommendations, and follow-up needs to primary care providers and health plans.
- Provide general recommendations to members to enhance their understanding of chronic conditions and recommended testing.
- Actively address and close identified gaps in care, whether disease-specific or preventive.
- Recognize and appropriately escalate emergent or urgent situations, taking action as per company policies and professional judgment.
- Perform, document, and communicate results of Point of Care (POC) Testing.
- Maintain strict compliance with Company policies, procedures, mission statement, and all HIPAA confidentiality requirements.
- Fulfill additional duties as reasonably assigned to achieve company operational and financial success.
- Adhere to responsibilities regarding separation of duties for applicable processes.
- Be available for a minimum of 25 slots (approximately 37 hours) per month.
Qualifications
- Possess an active, un-encumbered license to practice nursing.
- Hold ANCC or AANP board certification as a Nurse Practitioner or Physician Assistant specializing in Family, Adult, Gerontology, or Emergency Medicine.
- Maintain current CPR certification.
- Comply with recommended Healthcare Personnel Requirements for vaccinations and preventive testing prior to hire, including Hepatitis B, Influenza, MMR, Tdap, Varicella, and Tuberculosis.
- Prior Home Health experience is a plus.
- Exhibit excellent communication skills, especially with elderly and chronically ill patients and their families.
- Demonstrate understanding of Medicare, Medicaid, and Health Plan benefit structures.
- Possess the ability to multitask effectively.
- Deliver excellent customer service.
- Bi-lingual or multi-lingual skills are a plus.
- Ability to practice autonomously in a remote clinical environment, including independent patient assessments, formulating evidence-based treatment plans, managing complex chronic conditions, and making sound clinical decisions without direct on-site supervision.
