About the Role

The Case Management Specialist - Advanced Practice at Compass Connections plays a critical role in delivering crisis intervention, case management, and comprehensive assessments. The role requires fluency in English and Spanish and is ideal for those with a background in social work, psychology, or a related field.

Key Responsibilities

  • Meet federal and state regulatory guidelines and maintain caseloads.
  • Provide crisis intervention support and coordinate referrals.
  • Conduct comprehensive assessments, develop treatment plans, and document progress.
  • Participate in weekly case staffing meetings and treatment team reviews.
  • Travel extensively (75%), including overnight stays and varied transportation.
  • Adhere to Compass Connections safety protocols and maintain confidentiality.
  • Work evenings, weekends, and holidays as needed.

Qualifications & Requirements

Applicants must have a bachelor’s degree in social work, psychology, criminal justice, sociology or a related field, or have equivalent experience. Graduate credentials are preferred. Certifications required include Integrated Case Management, First Aid, CPR, and Emergency Behavior Intervention. A minimum of three years related experience is required, or seven years for those without the listed academic background.

Additional Information

Candidates must pass pre-employment drug screens and background checks. Candidates are expected to maintain extensive travel, computer literacy, and familiarity with Compass Connections policies. An annual Influenza vaccination or approved exemption is required.
