The Opportunity: Nurse Practitioner/Clinician
CHN is seeking a Family Nurse Practitioner for its East New York Health Center, located at 999 Blake Avenue, Brooklyn, NY 11208-3535.
Reporting to the Medical Director, the Nurse Practitioner/Clinician will be responsible for providing evidence-based Primary Care to patients. This role is an integral part of a collaborative care team, which includes nursing staff and other essential services such as Health Education, Nutrition, Social Work, and Behavioral Health.
Qualifications
The ideal candidate will possess:
- A valid Family Nurse Practitioner N.Y.S. License to deliver medical care.
- At least two (2) years of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
