We specialize in licensing software for managing medical programs, particularly the Medication Therapy Management (MTM) Program as mandated by CMS for Medicare Part D. Additionally, we provide outsourced MTM services, leveraging our expert team of pharmacists, clinicians, and support staff to conduct crucial Comprehensive Medication Reviews (CMRs) for our clients' members. These reviews are instrumental in maintaining high star ratings within Medicare Part D programs.

The Role: Clinical Interviewer - Pharmacist

The Clinical Interviewer - Pharmacist reports directly to the Supervising Pharmacist and is responsible for delivering individualized MTM services. This includes, but is not limited to, performing Comprehensive Medication Reviews, Adherence/Targeted Interventions, and Transitions of Care. The MTM Pharmacist will also play a key role in finalizing reviews completed by clinical staff members and assisting with Comprehensive Medication Review quality assurance.

Key Responsibilities

  • Deliver MTM services, including Comprehensive Medication Reviews, Adherence/Targeted Interventions, and Transitions of Care, to assess medication appropriateness and optimize therapy.
  • Perform accurate medication reconciliation and allergy reviews.
  • Provide patient education and counseling on common chronic disease states such as diabetes, hypertension, dyslipidemia, and COPD.
  • Assist patients in managing medication costs through interventions like formulary and therapeutic interchanges, pill splitting, and identifying unnecessary or duplicate therapies.
  • Assess prescribed medication therapy for unnecessary medications, therapy gaps, ineffective or harmful medications, inappropriate doses, adverse drug reactions, and adherence issues.
  • Collaborate with the interdisciplinary care team, patients, and/or caregivers to identify and develop patient-specific disease state therapy goals.
  • Determine appropriate interventions to resolve medication therapy problems, achieve therapy goals, and prevent new medication-related issues.
  • Finalize comprehensive medication reviews completed by clinical staff members (LPNs, RNs, pharmacy student interns).
  • Oversee call center operations and communicate with CSS staff members to ensure adherence to standard operating procedures.
  • Perform quality assurance reviews for clinical staff members, provide continuing medication education, and participate in clinical staff meetings.
  • Efficiently and accurately document all services provided.
  • Adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of federal or state laws.

Qualifications

  • BS in Pharmacy or PharmD from an accredited school of pharmacy.
  • Current license to practice Pharmacy in Arizona in good standing.

  • Knowledge of MTM Core Elements: Strong understanding of the 5 Core Elements of MTM services: Medication Therapy Review (Comprehensive and Targeted), Personal Medication List (PML), Medication-Related Action Plan (MAP), Interventions, and Documentation of Services.
  • Clinical Therapeutic Expertise: Strong clinical therapeutic skills and knowledge of appropriate medication dosing for geriatric and adult populations.
  • Autonomy & Collaboration: Ability to work independently and effectively within a collaborative environment.
  • Communication & Resolution: Strong telephonic, listening, and conflict resolution skills; ability to communicate clearly and empathetically with senior citizens, patients, and staff.
  • Professional Demeanor: Consistently maintain professional behavior, dependability, and reliability.
  • Self-Management: Self-motivated, organized, detail-oriented, with effective time and resource management.
  • Legal & Ethical Compliance: Knowledge of state pharmacy law and CMS compliance measures, ensuring privacy and confidentiality as required by HIPAA and company guidelines.
  • Computer Competency: Efficiently multi-task between several different computer applications.
