About the Role: Clinical Pharmacist, Ambulatory Neurology
ChristianaCare is actively seeking a Clinical Pharmacist to support our expanding, dual-accredited Specialty Pharmacy Program. This role is uniquely embedded within Neurology practices across the health system, offering a hybrid work arrangement that combines on-site collaboration with providers and virtual patient care across multiple neurology practice locations.
As a Clinical Pharmacist, Ambulatory Neurology, you will be instrumental in delivering high-touch, patient-centric specialty pharmacy services. You will serve as a trusted medication expert for both patients and neurology providers, playing a critical role in optimizing complex medication therapies, improving access, and enhancing the overall specialty pharmacy experience.
Key Responsibilities
- Serve as the primary pharmacy resource embedded within Neurology specialty practices.
- Provide comprehensive education and ongoing monitoring for specialty medications.
- Address medication-related questions and concerns between provider visits.
- Review and interpret medication orders, assessing appropriateness of therapy, efficacy, therapeutic goals, adverse effects, contraindications, and various drug interactions (drug–drug, drug–food, drug–lab, and drug–disease).
- Monitor and evaluate drug therapy outcomes in accordance with established clinical guidelines.
- Document patient encounters and follow-ups, adhering to URAC and ACHC accreditation standards.
- Collaborate closely with specialty pharmacists, technicians, access coordinators, and call center teams to ensure seamless prescription processing and patient support.
- Participate in quality improvement initiatives, medication use evaluations, and safety reporting.
- Contribute to the development and revision of pharmacy and health system policies and protocols.
- Provide education and training for pharmacy staff, allied health professionals, and patient groups.
- Precept pharmacy residents and students, and participate in research or scholarly activities as assigned.
Education & Experience Requirements
- PharmD from an accredited school of pharmacy required.
- Completion of an ASHP-accredited PGY1 residency or at least 3 years of clinical experience required.
- Board Certification (BPS) or Certified Specialty Pharmacist (CSP) preferred.
- Ambulatory care and/or specialty pharmacy experience preferred.
- Current licensure (or eligible to reciprocate) in the State of Delaware.
- Ability to obtain licensure in additional states as required for virtual patient care.
