Job Summary and Qualifications

The Clinical Pharmacist will provide comprehensive pharmaceutical services for hospital inpatients, ensuring safe and effective medication therapy. The role involves oversight of the medication use system, compounding and dispensing pharmaceuticals, monitoring drug interactions, and suggesting cost-effective therapeutic alternatives.

Responsibilities include:

• Interpretation of orders and transcription into the pharmacy computer system
• Accurate compounding and dispensing of pharmaceuticals
• Monitoring drug therapy for contraindications, interactions, and allergies
• Suggesting therapeutic medication alternatives to medical staff
• Overseeing hospital formulary and minimizing non-formulary procurements
• Assisting with clinical services such as drug consults and dosage monitoring
• Completing notification reports for medication errors and adverse drug reactions
• Providing oversight of staff schedules

Qualifications:

• B.S. or PharmD from an ACPE-accredited School of Pharmacy
• Active pharmacist license in good standing
• State Specific Medications Therapy Management Certification
• 1+ year of hospital clinical pharmacy experience or 2+ years in general hospital pharmacy
• Hospital PGY1/pharmacy general practice residency or fellowship preferred
• Specialty BCPS certification is a plus
