The Opportunity: Ambulatory Care Clinical Specialist Pharmacist - Inflammatory Bowel Disease
Cedars-Sinai Medical Center in Los Angeles, California is seeking an Advanced Practice Pharmacist specializing in Inflammatory Bowel Diseases (IBD). This role involves working as part of a multidisciplinary team to deliver comprehensive medication management for IBD patients within an ambulatory care setting.
Key Responsibilities
- Perform medication reconciliation and therapy monitoring.
- Actively manage patient side effects related to medications.
- Provide comprehensive education to patients on specialty medications and infusions.
- Assist patients with medication access and adherence.
- Achieve positive patient outcomes through evidence-based disease management.
- Initiate, adjust, or discontinue therapies based on current clinical guidelines (e.g., American College of Gastroenterology, Cedars-Sinai’s P&T-approved protocols).
- Order and interpret laboratory results relevant to patient care.
- Authorize medication refills efficiently.
- Serve as a clinical resource for physicians and support staff.
- Act as a liaison with the Cedars-Sinai Medical Center Specialty Pharmacy.
- Contribute to the development and maintenance of order sets, best practices, and protocols.
- Participate in competency assessments and ongoing quality improvement efforts.
Qualifications and Requirements
Educational Background:
- Graduate Degree: Graduate of an accredited School of Pharmacy – Required.
- Completion of ASHP accredited residency program – Preferred.
Licenses and Certifications:
- California Pharmacist license – Required.
- Advanced Practice Pharmacist License – Strongly preferred or in progress.
- Board certification in specialty pharmacy – Strongly preferred.
Additional Skills and Experience:
- Clinical Practice Experience: Previous experience in IBD specialty or infusion therapy management in an ambulatory care setting is preferred.
- Patient Experience: Excellent patient service skills, prioritizing patient care needs for safe and effective medication use.
- Team Skills: Ability to collaborate effectively with internal teams and stakeholders.
- Analytical: Strong critical thinking for analyzing and interpreting information, data, and making sound decisions. Exhibits good clinical reasoning skills in managing complex patients.
- Communication: Proficient in conveying and receiving information verbally and in writing across various audiences. Engages in positive inter/intra departmental interaction.
- Documentation: Accurate and prompt documentation of clinical interventions, assessments, and therapy changes in the Electronic Health Record (EHR).
- Project Management: Capable of initiating, planning, designing, implementing, and monitoring projects. Ability to function independently, organize, direct, and evaluate assigned work.
- Budgetary & Financial: Fiscal management skills to prepare, monitor, control expenses, and generate revenue.
- Regulatory: Ability to interpret and apply knowledge of State, Federal, and Agency standards.
- Technical: Adaptability to learn and use software, technology, and automated information systems.
Physical Demands:
- Frequent sitting, walking, bending, reaching. Occasional pushing, pulling, lifting materials up to 50 pounds.
- Responsiveness to auditory alarms and communication devices (telephones, pagers, fire alarms).
- Sufficient mobility to access equipment, patients, and other customers.
- Ability to read information on medication labels.
