Overview
CAN Community Health Inc., the nation’s premier resource in ending epidemics, seeks a Clinical Quality Improvement Coordinator (Remote). This role focuses on enhancing clinical quality in patient care, specifically for individuals impacted by HIV, Hepatitis C, STIs, and other infectious diseases.
Essential Duties
- Develop and implement systems for quality measure performance communication.
- Educate clinicians on best practices to improve quality outcomes.
- Identify and close gaps in care progress.
- Assist with workflow design and redesign for quality improvement.
- Develop comprehensive clinical quality improvement policies and procedures.
- Collaborate with stakeholders to align on quality initiatives.
- Compile data for special projects and audits.
- Lead monthly quality improvement meetings at clinics.
- Represent CAN at quality management meetings with grantor jurisdictions.
- Work with the EHR Team on quality dashboards and electronic data capture tools.
- Collaborate with the Ryan White Program Team on quality management plans.
Secondary Duties & Professional Development
Participate in ongoing professional training to advance knowledge in quality measures, value-based care, and payer requirements. Maintain a drug-free workplace and adhere to all professional, ethical, and HIPAA standards.
