Job Summary

The Clinical Pharmacist provides comprehensive pharmaceutical care to Baptist Health patients of all ages. This involves medication order review, preparation, dispensing, documentation, monitoring, patient counseling, and medication reconciliation. The Clinical Pharmacist collaborates extensively with patients, physicians, nurses, and other healthcare professionals to ensure the delivery of safe, effective, and fiscally responsible patient-centered care.

Key Responsibilities

  • Interpret, review, and process medication orders, screening for appropriateness and intervening as necessary.
  • Provide therapeutic assessments and selected disease management for assigned clinical areas.
  • Work towards improving both patient and practice outcomes.
  • Counsel patients and provide medication education.
  • Participate in medication reconciliation processes.
  • Ensure compliance with all regulatory and hospital policies.

Qualifications

  • Degrees: Bachelor of Science (required), Doctor of Pharmacy (Pharm D) preferred.
  • Licenses & Certifications: Registered Pharmacist (required).
  • Experience: No experience required. Progressive pharmaceutical care experience or Pharmacy Practice Residency preferred.
