Overview
The Clinical Pharmacist - MTM Chronic Disease Management at Ascension works on a PRN basis in the Pharmacotherapy Clinic at Ascension Saint Joseph in Chicago, IL. This role involves ensuring accurate medication selection, dosing, and dispensing in specialized areas such as oncology, cardiology, and nuclear medicine.
Responsibilities
- Review medication orders to prevent and resolve drug related problems.
- Recommend formulary alternatives and compound medications.
- Provide drug information and education to staff and patients.
- Communicate with physicians and oversee technical staff.
- Preceptor for pharmacy students and provide patient counseling.
- May float to cover additional pharmacy needs.
Requirements
Applicants must be credentialed by the Illinois Department of Financial and Professional Regulation, possess a Bachelor's or advanced degree in Pharmacy, and have relevant experience in ambulatory care through residency or direct work experience.
Additional Information
This PRN position supports chronic disease state management along with anticoagulation management and post-discharge counseling. Coverage options may be in-person or remote with no evening, weekend or holiday shifts required.
