The Program Manager, Care Transformation Role

The mission of a Program Manager, Care Transformation is to cultivate robust, long-term partnerships that instigate significant positive change for providers, patients, and administrators within the healthcare landscape. This role involves close collaboration with health system leaders to drive the adoption of Ambience solutions, from initial pilots to full-scale enterprise rollouts. The primary objective is to ensure Ambience delivers measurable clinical and financial outcomes, acting as a trusted advisor to align executive priorities with frontline needs and enable superior care delivery at scale.

What You'll Do

• Build high-trust partnerships with clinical, operational, financial, and IT leaders across your accounts, serving as the primary face of Ambience to customers.
• Lead the entire implementation process for Ambience solutions within complex health systems, coordinating workflow discovery, technical integration, customer onboarding, and go-live execution for rapid time-to-value.
• Collaborate closely with Product and Clinician Success teams to continually enhance the customer experience, providing structured feedback to influence roadmap decisions and develop adoption playbooks.
• Own the post-sale execution of Ambience pilots from initiation, ensuring they stay on track, deliver meaningful value quickly, and transition into long-term enterprise partnerships.
• Shape and present clear ROI narratives that resonate with health systems, supporting executive readouts and Quarterly Business Reviews to build strong advocacy.
• Accelerate time to value by proactively identifying and mitigating roadblocks, managing change effectively, and guiding partners through pilot-to-scale transitions with confidence.
• Contribute to the collective improvement of the Care Transformation team by documenting repeatable playbooks and pilot frameworks.

Who You Are

• You have 4+ years of experience in healthcare transformation, health tech implementation, customer success, or healthcare management consulting, with a deep understanding of provider organizations and complex health systems.
• You possess a proven track record of leading post-sale motions, converting pilot initiatives into sustained contracts, and fostering champions across clinical, operational, and executive levels.
• You are an exceptional communicator, adept at navigating between clinical and operational terminology to engage diverse stakeholders with clarity and credibility.
• You thrive in high-ambiguity environments, capable of creating structure, anticipating challenges, and taking ownership without constant direction.
• You are deeply aligned with the mission to improve healthcare, motivated by a shared urgency to address systemic issues.

This role requires up to 30% travel for onsite support during key customer events, stakeholder meetings, and QBRs across the U.S. All travel expenses are covered according to Ambience’s travel policy.
