Registered Nurse (RN) - Case Manager
Adaptive Home Health is seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join our growing team. We are committed to building a high-acuity, patient-centered, skilled home health model across Texas, aiming to dramatically improve patient access to home health care. As an RN Case Manager, you will be at the forefront of our mission, delivering skilled care in the home, advocating for patients and families, and helping us elevate the standard of home health.
About Adaptive Home Health
We leverage technology to enhance support for our field clinicians. Our charting product is designed to minimize documentation time, allowing for more efficient patient visits. A new scheduling product is launching soon to further improve coordination and protect clinician time.
This role offers autonomy, meaningful patient impact, and a supportive team dedicated to reducing administrative burdens for clinicians who excel at owning outcomes.
What You Will Do
- Own patient assessments and skilled visit documentation, including OASIS.
- Deliver high-acuity home health care, specializing in wound care and infusion therapy.
- Develop and maintain clear plans of care through consistent communication with physicians, care team members, patients, and families.
- Act as the patient's advocate, ensuring continuity of care across all disciplines.
- Manage case management responsibilities, including care coordination, timely follow-ups, and resolving barriers to patient progress.
- Communicate changes in patient condition, obtain and track orders, and coordinate services (PT/OT/ST/MSW/HHA) to ensure the plan of care is implemented.
- Administer medications and treatments as ordered, and provide essential patient and caregiver education.
- Coordinate with internal clinical and operational support teams to ensure reliable and safe care delivery.
- Provide supervision and guidance to LVNs and HHAs where applicable.
- Participate in case conferences and in-services to ensure alignment on quality, safety, and clinical outcomes.

Requirements
- Current Texas RN license.
- Minimum of 3 years of clinical nursing experience.
- Proficiency in wound care and infusion therapy.
- Current CPR (BLS) certification.
- Valid Texas driver's license and comfort with field-based work.
- Strong time management and organizational skills in an autonomous environment.
- Clear, professional communication with patients, families, and providers.

- Previous home health experience.
- Familiarity with Homecare Homebase (HCHB) or similar EHR systems.
- Experience coordinating care for medically complex patients.
