Adaptive Home Health is seeking a dedicated Full-time Physical Therapist (PT) to join our team. This field-based role involves providing essential home health services across Texas, focusing on patient-centered care and improving access to home health.

What You Will Do
- Manage your caseload as a case manager, taking accountability for the patient’s therapy plan, progression, and overall outcomes.
- Conduct comprehensive in-home assessments and deliver skilled physical therapy visits.
- Develop and implement clear plans of care focusing on gait/balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Establish measurable goals, track objective progress, and adapt treatments as needed.
- Coordinate with the multidisciplinary care team, communicating changes in patient condition and aligning on orders.
- Manage treatment and outcomes, including supervising PTAs when appropriate.
- Coach patients and caregivers on practical, sustainable home programs.
- Document patient encounters promptly and thoroughly using Homecare Homebase (HCHB).

Requirements
- Active Texas PT license in good standing.
- Strong clinical judgment and ability to work independently in the field.
- Comfortable escalating changes in condition to RN/MD and documenting rationale.
- Excellent communication and documentation skills.
- Reliable follow-through on orders, frequencies, and patient goals.
- Current driver’s license and willingness to travel daily within the assigned territory.
- Ability to carry and transport basic therapy equipment.
- Proficiency in core home health PT scope, including safety-first mobility work (transfers, gait training, balance/fall prevention, assistive devices).
- Experience in strengthening, functional mobility, and home safety assessment/modification education.
- Skilled in patient/caregiver coaching with practical home programs.
