Occupational Therapist (OT) - Adaptive Home Health
Adaptive Home Health is seeking a full-time Occupational Therapist to join our team. We are dedicated to improving patient access to home health care across Texas by providing a higher-acuity, patient-centered skilled home health model. This field-based role offers autonomy and significant patient impact.

What You Will Do
- Complete in-home OT evaluations and deliver skilled OT visits.
- Develop and execute care plans focused on ADLs/IADLs, safety/fall prevention, and functional independence.
- Recommend and train on adaptive equipment and home modifications.
- Coordinate with nursing and the care team regarding patient condition changes.
- Document promptly and thoroughly in Homecare Homebase (HCHB).

Requirements
- Active Texas OT license (in good standing).
- Strong clinical judgment and ability to work independently.
- Comfort escalating condition changes to RN/MD and documenting rationale.
- Excellent communication and documentation skills.
- Reliable follow-through on orders and patient goals.
- Current driver’s license and comfort with daily travel.
- Ability to transport basic therapy equipment.
- Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).
